Use the form below to sign up your child to play CYO athletics at St. Sebastian.  The information provided will only be available to the St. Sebastian Athletic Director, Booster Club board and the team coaches.  Once the sign up is complete the information will be saved by the Atheletic Director and deleted from the site.   You are only required to supply us with enough information to sign up your child, put them on the proper team and allow the coaches to contact you.  If this is the first sport you child is playing this school year download and fill out the CYO registration form found on the CYO forms page.
